Penetração intratubular após diferentes protocolos de ativação da irrigação endodôntica: estudo in vitro comparativo

Introdução: A ativação da irrigação endodôntica pode favorecer a penetração das soluções nos túbulos dentinários, potencializando sua ação no interior do sistema de canais radiculares. Objetivo: Avaliar e comparar, in vitro, a profundidade de penetração intratubular de um corante após diferentes protocolos de ativação da irrigação endodôntica. Metodologia: Foram utilizados 90 raízes distais de molares inferiores humanos extraídos, distribuídos aleatoriamente em seis grupos (n=15). Os canais radiculares foram instrumentados pela técnica crown-down com instrumentos rotatórios ProTaper Next até o diâmetro apical 40 e conicidade .06 (X4), utilizando motor X-Smart Plus. Durante o preparo, os canais foram irrigados com 2mL de hipoclorito de sódio (NaOCl) a 5% entre cada instrumento. Após o preparo, foi realizado o protocolo final de irrigação, composto por NaOCl 5% (5 mL/1 min), água ultrapura (5 mL/1 min), EDTA 17% (5 mL/1 min), com ativação durante 30 s, seguida de água ultrapura (5 mL/1 min) e NaOCl 5% (5 mL/1 min), com ativação por 30 s, repouso de 30 s e nova ativação por 30 s. Após irrigação com água ultrapura e respectiva secagem, os canais foram preenchidos com azul de metileno a 1%, submetido à ativação durante 30 s de acordo com o grupo experimental: irrigação convencional (IRC), irrigação sônica (IRS), irrigação com agulha dupla lateral (IRD), irrigação passiva ultrassônica (PUI), Easy Clean (ECL) e Smart Lite Pro (SLP). Posteriormente, as raízes foram seccionadas transversalmente nos terços cervical, médio e apical, e a média da profundidade de penetração do corante nos túbulos dentinários foi mensurada por microscopia com auxílio do software ImageJ. Os dados foram submetidos à análise estatística, considerando nível de significância de 5%. Resultados: Foram observadas diferenças significativas entre os protocolos de ativação nos terços cervical, médio e apical (p<0,001). No terço cervical, as maiores médias de penetração foram observadas para PUI (704,2 µm), SLP (692,4 µm) e IRS (643,5 µm), enquanto IRC (193,9 µm) e IRD (199,1 µm) apresentaram os menores valores. No terço médio, PUI (525,1 µm) e SLP (484,6 µm) apresentaram as maiores médias de penetração, sem diferença estatisticamente significativa entre ambos, enquanto o SLP apresentou penetração significativamente maior que o IRS (292,9 µm; p<0,001). No terço apical, PUI (141,9 µm) e SLP (131,9 µm) apresentaram as maiores médias de penetração, sem diferença estatisticamente significativa entre ambos, enquanto os demais grupos apresentaram valores inferiores a 100 µm. Conclusão: Os protocolos PUI e SLP proporcionaram maior penetração intratubular corante, enquanto IRC e IRD apresentaram os menores valores. A penetração diminuiu significativamente do terço cervical para o apical em todos os grupos. A ativação do irrigante, seja ultrassônica ou sônica, mostrou-se determinante para otimizar a penetração do corante nos túbulos dentinários.
